Insurance Information
If your dental insurer does not provide you with a card, please bring the name of the company, your account number, and any other pertinent information. It is the patient’s responsibility to find out what portion of their dental care is covered by their policy. Please contact them before moving forward with a procedure to ensure you are well-informed about their coverage and deductibles.
Dentist Referral Notes or Images
If your dentist has provided you with information regarding his or her findings or has printed images for you to bring to Dr. Zaremba-Rabatin, please remember to bring them to your appointment.
